No general minimum loss was found; the only threshold is that a claimant must be unable to work at least three days to get work loss benefits (N.D. Admin. Code 94-03-03-13(2)(c)). No relocation, security, or temporary housing benefit appears in the statute, rules, or program pages. The program page agrees with the statute on the $25,000 total cap, the $300 per week wage cap, the 96-hour report, and the one-year filing deadline; the page states the wage cap as 'up to 16 weeks' and the rule as a $4,800 maximum (same total). Filing deadline runs from the date of the police report, not the crime. Work loss is limited to four months from the crime and is not paid while sick or annual leave is available. Travel requires at least 150 miles round trip and vehicle rental is capped at $30 per day. If the claimant has no insurance or other collateral source, the program pays 80 percent of billed charges for eligible medical, dental, or mental health services (94-03-03-06(3)); the 2025 mental health policy form describes provider reimbursement at 80 percent of patient responsibility. Forensic exam: under N.D.C.C. 12.1-34-07 the acute exam (within 120 hours unless good cause) may not be charged to the survivor directly or through a third-party payer, and the Attorney General reimburses providers; the statute does not condition this on a police report. The program page notes crowdfunding may be treated as a collateral source and restitution or civil recovery must be repaid. Contributory misconduct rule 94-03-03-05 lists conduct that requires denial; Tend shows conduct rules as information only. Reviewer: N.D.C.C. 12.1-34-07(2) separately bars charging a child forensic exam (criminal sexual conduct, physical abuse, or neglect) to the child or parent, and is recorded as ND-EXAM-CHILD. How to file: North Dakota has no PDF application. The program page says the application is online only (portalapps.nd.gov/docr/cvc-application/), so application_form is null with fillable false and field_count 0; the portal page could not be saved (blocked or empty, also through Chrome), so the application's own field list comes from the CVC Application Tips PDF (ND-S6). No official source states a mail, email, or fax filing method for the application itself; the program page and admin rule 94-03-01 give P.O. North Dakota has no address confidentiality program (Minnesota SOS directory lists 'North Dakota No Program'; no ND statute or agency page found), so no address_confidentiality rule was added. The Attorney General's Marsy's Law page describes a victim's right to prevent disclosure of information that could be used to locate the victim; that is not a substitute-address program.
