How to check this page

Arizona's caps and eligibility rules are in the administrative code (A.A.C. R10-4-101 to R10-4-111, final rules effective July 3, 2024; snapshot is Supp. 26-1, March 31, 2026), not the statute. No minimum loss is set in the rules. The rules list no relocation or home security expense; transportation for responding to a substantiated safety threat is covered under R10-4-108(C)(6)(e), and crime scene cleanup (sexual assault included) covers repairing exterior doors, locks, or windows damaged in the crime at a residence (R10-4-101(8)(b)). No statewide alternative to the 72-hour police report was found for sexual assault; the only relief is the Board's good-cause extension. A.R.S. 36-420.02 bars extra fees and balance bills to the patient for county-paid forensic exams but does not address billing the patient's insurance. The lost-wage cap has no fixed dollar amount (40 hours a week at the current minimum wage), so amount_cents is null. You should not be billed for the exam

A hospital or provider may not send the patient a balance bill for a county-paid forensic exam. The statute does not say whether the patient's insurance may be billed.

The Secretary of State's Address Confidentiality Program gives survivors of sexual offenses, domestic violence, and stalking a substitute address and free mail forwarding so the person they fear cannot find them through public records. Eligible applicants include victims of a sexual offense such as rape or sexual assault. Enrollment is only through an in-person meeting with a registered Application Assistant at a victim services agency.

The Arizona Address Confidentiality Program (ACP) is a program that helps victims of domestic violence, sexual offenses, and/or stalking from being located by the person they are seeking safety from through public records. The program provides a substitute address and confidential mail forwarding services to individuals and families across Arizona.

Sources disagree on the filing clock: the undated ACJC brochure says to apply within two years of the crime, while A.A.C. R10-4-106(A)(7) says two years after discovery; the rule wins and is what this file uses. The application PDF linked from the ACJC page is dated June 2015 and predates the 2023/2024 rule rewrite; online filing is at acjc.hostedbykarpel.com. The program is county-run: each County Attorney's office has its own board and coordinator, and awards depend on money available to that county (R10-4-108(D)(2), (I)). A.R.S. 41-2407(E) and (F) also bar a provider who accepts program payment from billing the victim for the same service (unless the program cannot pay in full) and bar debt collection on bills included in a pending claim. Statute 13-1414 covers exams for offenses under 13-1404, 13-1405, 13-1406 and 13-3212 and dangerous crimes against children. Not used: the 2018 ACJC desk manual (predates current rules). Reviewer: the brochure also says the crime must be reported within 72 hours "of the crime"; R10-4-106(A)(4) says within 72 hours after discovery, and the rule wins. The brochure's blanket exclusion of property loss or repair is older and broader than the current rules, which pay for eyeglass/prosthetic/dental device repair (R10-4-108(C)(1)(a)(i)) and door, lock and window repair at a residence as crime scene cleanup (R10-4-101(8)(b)). R10-4-108(H)(1) requires denial for failure to cooperate with law enforcement unless the failure was due to a substantial medical, mental health, or safety risk. The forensic exam statute (A.R.S. 13-1414) covers only exam expenses arising from the need to secure evidence; follow-up medical treatment goes through the compensation claim. Address confidentiality (added Oct 3): the SOS eligibility page (AZ-S12) lists victims of a 'sexual offense (such as rape, sexual assault, molestation)' as eligible, plus fear for safety, a recent or planned move within Arizona to a place unknown to the perpetrator, and one piece of supporting documentation. Applications go only through a registered Application Assistant met in person (call 602-542-1653 to find one); approved participants are certified for five years and may not use the substitute address until the welcome packet and authorization card arrive. The ACP laws themselves (A.R.S. 41-161 to 41-169) were not snapshotted; the rule quotes the SOS program page. Record confidentiality: not found. A.A.C. R10-4-101 to R10-4-111 (AZ-S3) have no confidentiality section, and A.R.S. 41-2407 (AZ-S1) is silent. The application's release form (AZ-S7) says the opposite of a protection: information disclosed to the program is no longer subject to HIPAA, and the program may disclose non-identifying information for statistical purposes. County boards decide claims at open public meetings (Pinal, AZ-S14: 'The meetings are open to members of the public'; Maricopa says meeting recordings and documents are available for inspection, AZ-S10), so Tend should not tell Arizona users their claim details are confidential by law.
